Appointments for well child visits, sports physicals, camp physicals and school physicals may be requested using the PATIENT PORTAL and we will call you back.


For same day or urgent appointments, please do not use the patient portal as the messages are not seen right away. Call us at 602-840-3120, and select option 2 for our Scheduling Department.
