Insurance & Billing
We Accept Most Insurance Carriers
As there are many specific programs that any insurance company may be offering, we strongly recommend you contact your insurance company to ensure we are on their plan.
Always bring your insurance card to every appointment. Insurance companies and procedures change frequently, and our contracts with insurers require us to validate your insurance coverage each time we see you.
You are responsible for any co-pay, co-insurance, and/or deductible amounts due at the time of visit. If we are not contracted with our insurance we are still happy to treat your child; however, payment for services in full will be due at the time services are rendered. We can provide, at your request, our best estimate of charges prior to your appointment based on why your child is being scheduled.

Please also be aware that if your child is covered under any AHCCCS plans, we will be unable to see you even as a self-pay patient. AHCCCS rules do not allow for our providers to treat patients on a cash-pay basis.
